The following two forms are for your information only and do not need to be emailed as part of your registration. We would, however, ask that you prayerfully consider all of the given information before completing your registration.

The following four forms must be completed for each child each club year (beginning in August). Children in the same family may be combined on the registration form, the minor model release form, and the liability release form, but please have each child complete the participant agreement form separately. You may send the completed forms via email to: marciebharper@gmail.com.

Please bring club dues of $50 in cash or check payable to Kobayashi Maru to club meeting by September 29, 2023, or mail by September 25, 2023 to the above address.
